Poverty is one of the key social determinants of poor health. Reducing health disparities in underserved populations — which in urban America are typically populations of ethnic minorities — calls for wide and committed outreach efforts and culturally-inclusive primary care.
Ravenswood Family Health Center’s vision is to provide for every patient the experience of measurable improvement in their health outcomes, the belief that they have more control over their own health, and the feeling that the quality of their life has been enhanced.
Thanks in part to a capital donation from Sand Hill Foundation in 2010, Ravenswood Family Health Center was able to embark on a campaign to construct a 38,000-square-foot state-of-the-art medical facility to replace its aging modular clinic.
Thanks in part to a capital donation from Sand Hill Foundation in 2010, Ravenswood Family Health Center was able to embark on a campaign to construct a 38,000-square-foot state-of-the-art medical facility to replace its aging modular clinic. The new center in East Palo Alto has greatly expanded the clinic’s ability not only to deliver key healthcare services to the underserved communities of San Mateo and Northern Santa Clara counties, but also to increase educational opportunities for residents, provide access to new types of patient services, offer jobs to local residents, and much more.
